29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about March 2, 2016, and at times there prior to in transformer room at the Jamul Construction Project, the employer did not ensure flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. 1) One end of an extension cord connected to the spider box had insulation pulled away exposing employees to electrical hazard.
