29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, nor after any occurrence which could affect a scaffold's structural integrity: On or about March 3, 2016, at the jobsite located at 14191 Highway 94, Jamul, CA, the employer did not ensure that the tubular, welded scaffold system was not inspected by a competent person prior to the start of the work shift to identify deficiencies.

29 CFR 1926.451(f)(4): Any part of a scaffold damaged or weakened such that its strength is less than that required by paragraph (a) of this section shall be immediately repaired or replaced, braced to meet those provisions, or removed from service until repaired. On or about March 3, 2016, at the jobsite located at 14191 Highway 94, Jamul, CA the employer did not ensure that damaged or weakened scaffold parts were not repaired, replaced, braced or removed from service as required. Employees were working on a scaffold with damaged planks.
