29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about March 8, 2016 during the installation of masonry brick at the job site located at 7662 Rolling Green Ave NW, Massillon, Ohio 44646; the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects from the employees working directly above on a scaffold.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports.     On or about March 8, 2016, during the installation of masonry brick at the job site located at 7662 Rolling Green Ave NW, Massillon, Ohio 44646; the employer did not ensure employees working from a fabricated frame scaffold were protected from falling by ensuring all working platforms were fully planked or decked.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:   On or about March 8, 2016, during the installation of masonry brick 7667 Rolling Green Ave NW, Massillon, Ohio 44646, the employer did not ensure a supported scaffold (fabricated frame scaffold) was properly erected by not utilizing base plates to disperse weight of the scaffold on all end frames while employees were actively engaged in work upon the scaffold.
