29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources: Zeta Plus: On, or about, March 7, 2016, a maintenance technician suffered crushing injuries to two of his fingers when the platen on the Injection Molding Machine 16, an Autojector Model HCR-130 insert molding machine, fell onto his hands. Blocks, jacking blocks, wedges or other suitable hardware had not been provided for use at the machine during maintenance operations.
