29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) Lot #42, new residential construction located at1675 Bates Road, Alpharetta, GA 30009: Employees were exposed to eye injuries while operating pneumatic nail guns at 120 PSI without protective eyewear. Employees were exposed to struck-by hazards which included flying collated debris and fasteners as safety glasses and/or goggles were not provided.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:   (a) Cut benches on front and west sides, new residential construction located at 1675 Bates Road, Alpharetta, GA 30009: Employees were exposed to severe lacerations and amputation hazards while operating circular saws without a lower guard in place as a wood shim was placed between the guards which prevented the lower guard from retracting after a cut was made.

29 CFR 1926.451(b)(7): On scaffolds where platforms are overlapped to create a long platform, the overlap did not occur only over supports:  (a) West side, new residential construction located at 1675 Bates Road, Alpharetta, GA 30009: Employee was exposed to falls of approximately 27-feet while working from a ladder jack scaffold platform which was bridged with a 2x12 wooden plank and the bridge did not occur over a supporting member and the plank was not secured.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  (a) West side, new residential construction located at 1675 Bates Road, Alpharetta, GA 30009: Employees working from the ladder jack scaffold platform and the job made platform scaffold were exposed to falls up to 27-feet to the lower ground level as no fall protection system was used or provided by the employer.

29 CFR 1926.452(k)(1): Platforms exceeded a height of 20 feet (6.1 m):  (a) West side, new residential construction located at 1675 Bates Road, Alpharetta, GA 30009: The employer used a ladder jack scaffold at heights over 20 feet from the lower level which is prohibited. Employees worked at heights of approximately 27-feet from the lower level and at this height a different type of scaffold should have been used, i.e. pump jack scaffold.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Front porch roof, new residential construction located at 1675 Bates Road, Alpharetta, GA 30009: Employees were exposed to falls of approximately 13-feet, 5-inches while working from a 4/12 pitched roof installing cornice boards with no fall protection system in place.
