29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.    (a) West wing of building, roof, during roof sheathing and framing operations:  On or prior to 3/10/16, the employer did not certify that employees had been trained in recognition and avoidance of fall hazards associated with residential construction activities.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    (a) West wing of building, roof, during roof sheathing and framing operations:  On or prior to 3/10/16, four of five employees were not wearing protective helmets for protection from overhead and struck-by hazards.    Lee Contractors, LLC. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1025427, citation number 1, item number 1, and was affirmed as a Final Order on April 27, 2015, with respect to a workplace located at 277 County Road, Tenafly, NJ.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) West wing of building, roof, during roof sheathing:  On or prior to 3/10/16, employee(s) engaged in installation of OSB sheathing were exposed to fall hazards(s) of approximately 42' to ground from the edge of the roof deck and approximately 10' to 23' to the interior of the building.  An employee was observed working with harnesses worn incorrectly and not connected to a life-line system.     (b) West wing of building, 3rd floor exterior deck, during framing:  On or prior to 3/10/16, employee(s) engaged in framing operations were exposed to fall hazard(s) of approximately 32 to 40 feet from the exterior of the building.   Three employees were observed working without any fall protection system in use.      Lee Contractors, LLC. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1025427, citation number 1, item number 3, and was affirmed as a Final Order on April 27, 2015, with respect to a workplace located at 277 County Road, Tenafly, NJ.
