29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   The employer is failing to protect employees from potential fall hazards associated with conducting roof repairs on the Whispering Ridge Apartments located at 17551 Pinkney St. Omaha, NE 68116 when the employees of the companys subcontractor were lifted by the companys site foreman, three stories onto the roof using an aerial lift to make the repairs without using any form of fall protection.
