On or about March 9, 2016, at the above addressed jobsite, an employee performing masonry operations was exposed to falls when working on a tubular welded frame scaffold, eleven (11) feet three (3) inches above the ground, that did not have the working level fully planked.

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a. On or about, March 9, 2016, at the above addressed jobsite, an employee conducting masonry operations from a tubular welded framed scaffold, eleven (11) feet three (3) inches above the ground, was exposed to fall hazards due to the lack of a guardrail system at the sides and ends of the scaffold platform.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as used as a means of access. a. On or about March 9, 2016, at the above addressed jobsite, an employee conducting masonry operations from a tubular welded scaffold, eleven (11) feet three (3) inches above the ground, was not provided with an access ladder, thereby exposing the employee to falls.
