29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) 2074 Limestone Road, Wilmington, DE-On or about March 10, 2016, the contractor failed to provide guardrails to protect the employees from a fall hazard of approximately 15 feet above ground level. b) 2074 Limestone Road, Wilmington, DE-On or about March 10, 2016, the contractor failed to provide an effective personal fall arrest system to protect the employees from a fall hazard of approximately 15 feet above ground level.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: 2074 Limestone Road, Wilmington, DE-On or about March 10, 2016, the employer failed to train employees exposed to a fall hazard in the correct procedure, use and operation, and how to erect a guardrail systems, personal fall arrest systems, warning line, safety monitoring systems and for the inspection of fall protection equipment.
