Two employees were exposed to the hazard of falling 12'-0" while working from an aerial lift (i.e. Palm Beach Rental, Model TB-37D, Serial #FB06013) without fall protection. b. On or about March 10, 2016, at the west side of the building located at 4099 Tamiami Trail North, Naples, FL. Two employees were exposed to the hazard of falling while working from an aerial lift (i.e. Palm Beach Rental, Model TB-37D, Serial #FB06013) without fall protection.
