WORKSHOP:  The hazardous energy controls procedures did not includ step-by-step energy control procedures and/or alternative measures in the machine specific for the employees during servicing and maintenance of equipment, such as (but not limited to) the Matsurra M twin spindle CNC machining center.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    WORKSHOP: The operators of the Pre-Matsuura M twin spindle CNC machine were not protected from the point-of-operation while performing cleaning chips accumulated in and around the point-of-operation area with a hand-held air hose while the machine was in operating cycle.    On/about March 2, 2016, one of operators was cleaning chips accumulated in and around the point-of-operation area with an air hose while the machine was in operating cycle, and while performing the cleaning task, one of hands hit by rotating spindles/cutting tools and sustained nail/finger amputation from the incident.    ABATED DURING THE INSPECTION
