29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about March 11, 2016, at the above addressed job site, employees who had not been provided training to recognize fall hazards were exposed to a fall hazard of about 9 feet while working on an elevated surface constructing formwork without a means of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about March 11, 2016 , at the above addressed job site, employees were exposed to a fall hazard of approximately 9 feet while using an extension ladder that was not 3 feet above the upper roof landing and lacked a grasping device.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about March 11,  2016, at the above addressed job site, employees were exposed to a fall hazard of approximately 9 feet while working on an elevated surface constructing formwork without a means of fall protection.  The MBR Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR, 1926.502(b)1), which was contained in OSHA inspection number 315356618, citation number 1, item number 2 and was affirmed as a final order on March 16, 2012, with respect to a workplace located at 1020 NW 51 Street, Fort Lauderdale, FL 33309

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  On or about March 11, 2016, at the above addressed job site, employees were exposed to struck-by and caught-in-between hazards by being in a construction area where a powered industrial truck had been operated by employees who had not been trained and certified.
