New York Hyperbaric and Wound Care Center, LLC DBA Restorix Health, 215 Holland Street, Erie, PA - On or about March 8, 2014, and times prior thereto: Employees had occupational exposure to bloodborne pathogens intermittently throughout each day, when for instance, cleaning hyperbaric chambers, changing dressings, or cleaning blood or other potentially infectious material (OPIM) spills. A written Bloodborne Pathogen Exposure Control Plan had not been developed for employees of New York Hyperbaric and Wound Care Center, LLC DBA Restorix Health.

The employer had not developed and implemented a written Hazard Communication program.
