29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: On or about March 11, 2016, the employer failed to provide standard guardrails and toeboards on the new injection mold machine (Netstal Synergy 5000, Line #10) when an employee climbed upon the machine at a height of approximately 8 feet and was using the machine as a working platform while troubleshooting the machine and trying to clear a jam.

29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance: On or about March 11, 2016, the employer failed to ensure that an employee who was tasked with performing Lock Out Tag Out/Control of Hazardous Energy Control, as an Authorized Employee, was trained as an Authorized employee in the Control of hazardous Energy. This employee performed Lock Out Tag Out and maintenance work on machine #305 during the week of March 7, 2016.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about March 11, 2016, the employer failed to ensure that an injection mold machine located in the rear of the main facility was locked and tagged out prior to an employee performing troubleshooting and repair of the machine. On March 7, 2016, an employee received an amputation of his right middle finger tip while he was performing service/maintenance/troubleshooting on the Netstal Synergy 5000 Line #10 which was not Locked and Tagged Out.
