29 CFR 1910.253(e)(5)(iv): Hose connections were not clamped or otherwise securely fastened in a manner that would withstand, without leakage, twice the pressure to which they would normally be subjected in service, but in no case less than a pressure of 300psi:   a)    Precon Marine, Yard: On or about 03/11/2016, employees were observed on a barge using an oxygen hose that was repaired at the joint and not pressure tested to at least 300 psi as a result employees were exposed to possible burns.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15, 20 and 30-ampere receptacle outlets that are not part of the permanent wiring of the building structure and that are in use by personnel did not have ground-fault circuit-interrupter protection for personnel.  a)    Precon Marine, Shop: On or about 03/14/2016, an employee was not provided with a ground fault circuit interrupter device while plugged into the receptacle outlet on a Lincoln welder thus exposing the employee to a possible electric shock hazard.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1915.14(b)(1)(iv): Hot work was permitted in or on vessels and vessel sections, for which a Marine Chemist or Coast Guard authorized person certificate was not required under paragraph (a)(1)(iv) of this section, before being tested by a competent person and determined to contain no concentrations of flammable vapors equal to or greater than 10 percent of the lower explosive limit.   a)    Precon Marine, Barge: On or about 03/11/2016, employees conduct hot work on the topsides of barges that were not tested and certified safe for hot work by a Competent Person.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

No abatement certification or documentation required.
