29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (Construction Reference 1926.103)  a) 377 Route 17 South, Hasbrouck Heights, NJ 07604  An employee wore a tight fitting half face respirator while conducting concrete floor grinding for the purpose of leveling the floor. The floor contained 20% crystalline silica, and engineering controls were not in place, despite the presence of wet engineering controls integrated into the machine. The employer did not establish a written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i)-(ix), on or about March 9, 2016.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
