29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a. 309 Williams Avenue, Hasbrouck Heights, NJ 07604: An employee was permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: An employee working on an aluminum ladder was removing shingles within approximately 1-2 feet of the electrical service drop for the house, on or about March 11, 2016. Additionally, the employee carried materials up the ladder to employees working on the roof.       There is no abatement certification or documentation required for this item

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:      a) 309 Williams Ave, Hasbrouck Heights, NJ 07604: Employees were working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. Employees working on the lower level of the roof were exposed to a struck by hazard from roofing materials and equipment on or about March 11, 2016.    A.P.H. Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in inspection number 974958, citation number one, item number one and was affirmed as a final order on 6/25/14 with respect to a workplace located at 304 East Glen Ave, Ridgewood, NJ 07450.    There is no abatement certification or documentation required for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a) 309 Williams Avenue, Hasbrouck Heights NJ 07604: Employees were engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501. Employees removing and replacing a roof between approximately 12-30 feet above the ground were working without a personal fall arrest system or other means of fall protection, on or about March 11, 2016.    A.P.H. Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in inspection number 974958, citation number one, item number three and was affirmed as a final order on 6/25/14 with respect to a workplace located at 304 East Glen Ave., Ridgewood, NJ 07450.
