Additionally, the employee carried materials up the ladder to employees working on the roof. There is no abatement certification or documentation required for this item

Employees working on the lower level of the roof were exposed to a struck by hazard from roofing materials and equipment on or about March 11, 2016. A.P.H. Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in inspection number 974958, citation number one, item number one and was affirmed as a final order on 6/25/14 with respect to a workplace located at 304 East Glen Ave, Ridgewood, NJ 07450. There is no abatement certification or documentation required for this item.

Employees removing and replacing a roof between approximately 12-30 feet above the ground were working without a personal fall arrest system or other means of fall protection, on or about March 11, 2016. A.P.H. Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in inspection number 974958, citation number one, item number three and was affirmed as a final order on 6/25/14 with respect to a workplace located at 304 East Glen Ave., Ridgewood, NJ 07450.
