29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: Location: Building 4 On or about 3/11/16, Employees were exposed to a 16 foot fall hazard while loading masonry block on a fabricated frame scaffold that did not have guard rails around the interior of the scaffold.

29 CFR 1926.452(c)(5)(iii): Brackets used to support cantilevered loads shall only be used to support personnel, unless the scaffold has been designed for other loads by a qualified engineer and built to withstand the tipping forces caused by those other loads being placed on the bracket-supported section of the scaffold. Location: Building #4, On or about 3/11/16, side brackets were used to support a stairway on fabricated frame scaffold that was not designed by a qualified engineer.
