29 CFR 1910.1030(d)(3)(x): Masks in combination with eye protection devices, such as goggles or glasses with solid side shields, or chin-length face shields, were not worn whenever splashes, spray, spatter or droplets of blood or other potentially infectious materials may be generated and eye, nose or mouth contamination can be reasonably anticipated:     On or about March 11, 2016, employees administering medication were  exposed to occupational exposure to blood and other potentially infectious materials (OPIM).  The employer failed to ensure that employees were provided the appropriate personal protective equipment, such as eye protection and masks, to protect against splashes, sprays, and/or droplets.        Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure:    On or about March 11, 2016, employees administering medication were exposed to occupational exposure to blood and other potentially infectious materials (OPIM). The employer failed to ensure that the hepatitis B vaccine and vaccination series were available and offered to employees.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1030(h)(5)(i): The employer shall establish and maintain a sharps injury log for the recording of percutaneous injuries from contaminated sharps. The information in the sharps injury log shall be recorded and maintained in such manner as to protect the confidentiality of the injured employee. The sharps injury log shall contain, at a minimum:  A) The type and brand of device involved in the incident; B) The department or work area where the exposure incident occurred; and C) An explanation of how the incident occurred.   On or about March 11, 2016, the employer failed to maintain a sharps log or provide all of the equivalent information on the OSHA 300 logs, such as the brand of the device, the department where the exposure occurred, and an explanation of how the incident occurred, whenever a needlestick injury took place.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
