29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards:  a) On or about March 9, 2016 and times prior; The 330-1 forklift: The employer allowed the forklift operator to climb onto the wheel fender then onto the mast in order to get dunnage off the top of a stack which exposed him to the hazards associated with falls up to 5 feet 4 inches onto metal and/or the ground.

29 CFR 1910.24(f): Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs:  a) On or about March 9, 2016 and times prior; Plainer Mill, fixed stairs for stacker: The employer failed to ensure that concrete stairs were maintained is serviceable condition in that several stair treads had broken / missing concrete which could cause hazards associated with slip / trip / fall.  b) On or about March 9, 2016 and times prior; Saw Mill, fixed stairs leading up to mill: The employer failed to ensure that a concrete step was maintained is serviceable condition in that the second from the bottom step tread had broken / missing concrete which could cause hazards associated with slip / trip / fall.
