29 CFR 1904.40(a): Employer did not provide records kept under Part 1904 to an authorized government representative within four (4) business hours:    (a) On March 9, 2016, Lubricar, Inc., dba/Jiffy Lube did not provide copies of requested injury and illness logs within the allowed time frame. OSHA 300 and 300-A logs for years 2013, 2014,and 2015 were requested both verbally and in writing. Incomplete injury/illness reports were received two weeks after the inspection.       Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.303(g)(1)(i): Work space was less than 30 inches wide in front of electric equipment operating at 600 volts, nominal, or less:  (a) On or about March 9, 2016, and at times prior, Lubricar, Inc., dba/Jiffy Lube did not ensure that electrical equipment had a work space clearance of at least 30 inches wide in front of such equipment. Access to an electrical panel in the main shop was blocked by cleaning supplies, equipment, and a 55-gal drum.   Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v):    (a) On or about March 9, 2016, and at times prior, Lubricar, Inc. dba/Jiffy Lube, did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by the standard.  Employees were exposed to contact with chemicals hazards in the following areas:         1- In the lower bay, a 55-gal drum with Mighty Clean degreaser not labeled.      2- Throughout the shop, multiple 1-liter spray bottles containing Mighty Clean degreaser not labeled.      3- In the lower bay, a 300-gal containing windshield washer fluid had defaced labels.      4- In the lower bay, a 160-gal drum containing used coolant did not have hazard labels.
