29 CFR 1910.146(d)(3)(iii): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including isolating the permit space:  a) Voith Industrial Services 650 Miller Rd. Avon lake, OH, 44012: On or about March 4, 2016 the employer did not isolate the permit confined space water test pits, located in Apple 9, in that the double block and bleed isolation technique used to lock out the water supply system was improperly utilized in that the employer did not lock or tag both in-line valves and lock or tag the drain or vent valve in the line between the two closed valves exposing employees to a drowning hazard should the permit confined space fill with water.
