On or about March 11, 2016, at the jobsite where employees were exposed to struck-by hazards when cleaning around existing sewer lines and a manhole in an excavation without moving the spoil pile and excavator more than 2 feet from the edge.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about March 11, 2016, at the jobsite where employees were exposed to struck-by hazards when cleaning around existing sew lines and an existing manhole in an excavation greater than 5 feet in depth without using a cave-in protection system.
