29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by the point of operation and discharge area of compactors.      a) Work floor of warehouse - On or about March 14, 2016, employees were exposed to crushing injuries within the dump area of a green trash compactor, as the compactor's gate interlock was removed and bypassed.    b) Work floor of warehouse - On or about March 14, 2016, employees were exposed to crushing injuries within the stacking area of a brown cardboard compactor, as the compactor's gate interlock was removed and bypassed.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    a) Work floor main electrical panels - On or about March 14, 2016, employees were utilizing a main circuit service panel with the missing circuit breakers covered by tape, with live parts energized to 220 volts AC.
