29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  (a) Facility, 24 Frontier Drive, Gibsonia, PA:  On or about 03/14/16 the employer had not conducted training and evaluation of powered industrial truck operators to ensure they are competent to operate the equipment safely.  On 03/07/16 an employee suffered a broken right foot when it was caught between two powered pallet jacks.  The employee and other operators  who work in the warehouse had not been trained and evaluated by the employer to ensure they are competent to operate the equipment in a safe manner.
