29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  On or about March 14, 2016, the employer failed to provide the information contained in Appendix D of the Respiratory Protection Standard to employees who are permitted to voluntarily wear respirators. Employees wear half-mask respirators daily when working with acetone, and weekly when painting product. Employees wear dust masks daily while cleaning.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  On or about March 14, 2016, the employer failed to establish and implement the required elements of a written respiratory protection program for employees who voluntarily use respirators. Employees wear half-mask respirators daily when working with acetone, and weekly when painting product.

29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent.  On or about March 14, 2016, the employer, Tek Group International Inc., at the facility located at 567 Elm Ridge Road. Canal Fulton, Ohio 44614. failed to keep OSHA 300 logs or equivalent forms for recording injuries and illnesses for the calendar year/s 2016, 2015, 2014, and 2013.   Abatement Certification and Documentation is required.
