The outlet showed visible signs of shorting or overloading in that it was charred on the outer cover.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: On or about March 14, 2016, the electrical control panel located in the maintenance area hallway did not contain sufficient workspace around the equipment. The panel was blocked by a mounted rinse/ drain basin.
