OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to crushing hazards.          a) On or about February 27, 2016, at a jobsite located at 4100 S. Ashland Ave. Chicago, IL, the employer did not ensure that employees who were operating a John Deere Gator Truck to maintain the grounds of the facility were protected from tip over of the truck. There were three employees riding in a the two passenger truck, of which, one employee was riding on the hitch of the truck while the truck was in motion, which exposed them to crushing hazards caused by tip over of the truck. The employer did not ensure that employees were trained on the operation of the truck.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.    a) On or about February 27, 2016, the employer failed to report to OSHA the hospitalization of one or more employees as a result of a work-related accident.
