29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a)  BIRDSBORO KOSHER FARMS FACILITY - Employees performed servicing and maintenance operations on chicken processing and production equipment including but not limited to the AMFEC Mixer Model # 510, the chamber machine, flutie machine and frank-o-matic and the employer had not developed machine specific lock out tag out procedures detailing the appropriate procedure to lock and tag the equipment out of service, thereby exposing employees to serious injury, on or about March 2, 2016.  Birdsboro Kosher Farms Corp. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 951711, citation number 2, item number 1 and was affirmed as a final order on October 03, 2014, with respect to a workplace located at 1100 Lincoln Road, Birdsboro, PA 19508.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:   a)  BIRDSBORO KOSHER FARMS FACILITY - During repair and service activities, employees performed servicing and maintenance operations on production equipment including but not limited to the AMFEC Mixer Model # 510, the chamber machine, flutie machine and frank-o-matic and the employees had not been provided authorized employee lock out tag out training, thereby exposing employees to serious injury, on or about March 2, 2016.    Birdsboro Kosher Farms Corp. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 951711, citation number 1, item number 2a and was affirmed as a final order on October 03, 2014, with respect to a workplace located at 1100 Lincoln Road, Birdsboro, PA 19508. 29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   a)  BIRDSBORO KOSHER FARMS FACILITY - The employer did not conduct periodic inspections of the lock out tag out procedures on production equipment including but not limited to the AMFEC Mixer Model # 510, thereby exposing employees to hazards related to the unexpected start up or release of stored energy, on or about March 2, 2016.  Birdsboro Kosher Farms Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(6)(i), which was contained in OSHA inspection number 951711, citation number 1, item number 2b and was affirmed as a final order on October 3, 2014, with respect to a workplace located at 1100 Lincoln Road, Birdsboro, PA.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  a)  BIRDSBORO KOSHER FARMS FACILITY - Employees were performing preventative maintenance tasks such as adjusting and lubricating machines and corrective maintenance tasks such as clearing jams and repairing broken parts on machinery located in the production facility without affixing lock-out or tag-out devices to energy isolation devices, on or about March 2, 2016.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
