29 CFR 1926.416(a)(1): Employees were permitted to work in such proximity to any part of electric power circuits and could contact the electric power circuit in the course of work, and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means.  (a) Wayne, WV:  On or about March 9, 2016 and times prior, two employees were being lifted up to a roof to perform framing work under an overhead primary electrical service line, exposing the employees to the 200 amp primary service power line.    (b) Wayne, WV:  On or about March 10, 2016 and times prior, two employees were working on the outer edge of a building performing framing activities next to a power drop and weather head, exposing the employees to a primary service drop of 200 amps.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment are used to support scaffold platforms when they are not specifically designed by the manufacturer for such use.    (a) Wayne, WV:  On or about March 9, 2016 and times prior, two employees performing framing activities were being lifted up to a roof and working off of 4 foot wide and 8 foot long sheet of plywood that was attached to a wooden pallet.  The platform was not designed by the manufacturer to use as a scaffold platform for the Caterpillar TH514 Telehandler, exposing employees to falls of up approximately 19 foot, 4 inches from eave to uneven ground.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    (a) Wayne, WV:  On or about March 9, 2016 and times prior, employees using nail guns during framing activities were wearing sunglasses that did not offer protection against flying fragments from nails or other materials, exposing employees to eye injuries.      The employer, Asphalt Contractors & Site Work Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard Eye and Face Protection, which was contained in OSHA inspection number 775961, citation number 1, item number 1, and was affirmed as a final order on March 15, 2013, with respect to a workplace located in Madison, West Virginia.
