29 CFR 1926.754(e)(5)(i): Metal decking will be laid tightly and immediately secured upon placement to prevent accidental movement or displacement.    Employees performing decking activities on Building 5 were not securing the decking panels as they were being laid, exposing employees to a 39 foot fall hazard to the concrete below.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.      Employer did not ensure all employees performing decking activities were wearing their personal fall arrest systems and were tied off to protect the employees from a 39 foot fall hazard to the concrete below.
