29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  a. Site- Employees operating a DeWalt DW745 table saw are not protected from amputation and severe laceration hazards where the saw is operated without the blade guard. On or about January 11, 2016 an employee was operating the saw without the blade guard and sustained a severe laceration requiring them to be hospitalized.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:    a. Site- On or about January 11, 2016 an employee sustained a severe laceration which required the employee to be hospitalized for multiple nights. The employer failed to notify OSHA of the in-patient hospitalization.
