29 CFR 1926.502(d)(6)(i): A snaphook not designed for the following connections was engaged directly to webbing, rope or wire rope: a) Building 6 - On or about March 13, 2016 and at times prior thereto, employees were exposed to fall hazards from inadequately using their personal fall arrest systems. The lifeline was not attached or anchored according to the manufacturer, specifically the lifeline was wrapped around the anchor and attached to itself by the snap hook, creating the possibility for roll-out in the snap hook.

29 CFR 1926.502(d)(6)(v): A snaphook not designed for the following connections was engaged to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself: a) Building 6 - On or about March 13, 2016 and at times prior thereto, employees were exposed to fall hazards from inadequately using their personal fall arrest systems. The lifeline was not attached or anchored according to the manufacturer; specifically the lifeline was wrapped around the anchor and attached to itself by the snap hook. Also, there was an additional d-ring attached to the snap hook, creating the possibility for roll-out in the snap hook.
