29 CFR 1910.178(l)(4)(ii)(D): Refresher training in relevant topics was not provided to the operator when the operator was assigned to drive a different type of truck:  a)    Davis Boat Works: On or about 03/15/2016, employees were not provided refresher training prior to operating the Genie GTH 644 forklift trucks as a result an employee was severely injured.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
