29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    The employer did not ensure point of operation guarding was in place to protect employees from having any part of their body in the danger zone during operation.         a)   This violation was observed on or about March 15, 2016, the iron worker points of operation were not guarded exposing employees to hazards associated with being caught in the machinery.      b)   This violation was observed on or about March 15, 2016, the shear point of operation was not guarded exposing employees to hazards associated with being caught in the machinery.      c)   This violation was observed on or about March 15, 2016, the set of rollers located outside on the south side of the building was missing the emergency trip wire exposing employees to hazards associated with being caught in the machinery.      d)   This violation was observed on or about March 15, 2016, the chop saw located outside on the south side of the shop was not guarded exposing employees to hazards associated with being caught in the machinery.      e)   This violation was observed on or about March 15, 2016, the drill press point of operation was not guarded exposing employees to hazards associated with being caught in the machinery.      f)   This violation was observed on or about March 15, 2016, the chop saw located in the middle of the shop was not guarded exposing employees to hazards associated with being caught in the machinery.      g)   This violation was observed on or about March 15, 2016, the Cincinnati press brake located in the shop was not guarded exposing employees to hazards associated with being caught in the machinery.
