Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls from portable ladders:    At the facility, the employer did not ensure that fiberglass portable step ladders in the workplace and available for employee use were utilized in a safe manner.  On or about April 20, 2016, an employee was observed over reaching while facing away from the Werner 10-foot step ladder, labeled BP #2, to clean glue off of the rollers of the Retroflex drying station #20094299.   The employee was exposed to an approximate seven foot fall hazard.      Among other methods, one feasible and acceptable abatement of this hazard is to follow the ladder manufacturer's safety labeling for proper climbing and use.      Disclaimer:  a)  The employer is not limited to the abatement method(s) suggested by OSHA.  b)  The employer is responsible for selecting and implementing an effective abatement method.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:    Designated emergency exits for working at the south side of the production facility were obstructed:    a)  The conveyor of the Mark Andy press obstructs access to the receiving dock exit.  b)  A tote of paper waste obstructs access to receiving dock exit.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the employer's energy control procedures required by this standard:    Operators are exposed to the hazards of moving machine parts and in-going nip points while they set up and re-web printing presses, clean press rollers, and adjust slitters and blades during press operation.  The employer failed to implement energy control application steps as the machines are not shut down or turned off to perform the servicing work.  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], stored energy [(d)(5), if applicable], and verification of isolation [(d)(6)], are not implemented to safeguard employees from the machine servicing hazards:    1. This violation occurred on or about March 9, 2016 when an employee was allowed to perform work near the ingoing nip points created by the rollers of the Allied Gear printing press, Employer Identification #107, m/n 516 6N, s/n 971032.  The operator caught her finger in the ingoing nip point created by the perf roller while attempting to re-web the flexographic printing press using the jog controls which resulted in a partial amputation of the finger.      2.
