29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Facility, 450 Riverside Drive, Wyalusing, PA - Periodic inspections, at least annually, of the lockout tagout procedures utilized by employees prior to the performance of service and maintenance for equipment such as but not limited to the Darly Press Brake (440), Cincinnati Hydraulic Shear, Niagara Full Revolution Mechanical Power Press (F011) and Stone Machinery Co. Stone Saw (Serial No. 0307) were not conducted exposing employees to the unexpected release of energy in the event of equipment modifications, on or about March 15, 2016.
