The guard opening of the slitter machine was 6-5/8-inches high.  The guard opening was too large, allowing employee's hands / arms to enter into the in-running rollers.    B.)  Hanna Transfer Company, Inc. dba Moonlight Packaging, 99 Darr Street, Oil City, PA  16301, Building A, on or about August 26, 2016 and times prior thereto:  Employees operate a band saw to perform operations including, but not limited to, cutting cardboard.  A rotating bearing for the upper band wheel on the outfeed side of the bandsaw was 76.5-inches above the floor surface and was not effectively guarded.
