29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    A.)  Hanna Transfer Company, Inc. dba Moonlight Packaging, 99 Darr Street, Oil City, PA  16301, Building A, on or about August 26, 2016 and times prior thereto:  Employees stand and feed cardboard sheets into the infeed side of a slitter machine.  The guard opening of the slitter machine was 6-5/8-inches high.  The guard opening was too large, allowing employee's hands / arms to enter into the in-running rollers.    B.)  Hanna Transfer Company, Inc. dba Moonlight Packaging, 99 Darr Street, Oil City, PA  16301, Building A, on or about August 26, 2016 and times prior thereto:  Employees operate a band saw to perform operations including, but not limited to, cutting cardboard.  A rotating bearing for the upper band wheel on the outfeed side of the bandsaw was 76.5-inches above the floor surface and was not effectively guarded.

Hanna Transfer Company, Inc. dba Moonlight Packaging, 99 Darr Street, Oil City, PA  16301, Building A, on or about August 26, 2016 and times prior thereto:  Employees stand and retrieve cardboard sheets from the outfeed side of a slitter machine.  The guard opening was too large, allowing employee's hands / arms to come into contact with the rotating slitters.

Hanna Transfer Company, Inc. dba Moonlight Packaging, 99 Darr Street, Oil City, PA  16301, Building A, on or about August 26, 2016 and times prior thereto:  Employees operate a band saw to perform operations including, but not limited to, cutting cardboard.  A rotating power transmission shaft that was 23-inches from the floor was not guarded.

Hanna Transfer Company, Inc. dba Moonlight Packaging, 99 Darr Street, Oil City, PA  16301, Building B, on or about March 14, 2016 and times prior thereto:  Employees perform service and maintenance on equipment including, but not limited to, a 60-inch press.  The company did not conduct periodic inspections on the company's energy control procedures annually with authorized employees.  The last periodic inspection was conducted in February 2013.
