Three liquefied petroleum gas cylinders, one of which was upside down, were not secured near a fence by gate 1 on the northwest side of the facility.

29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: a. As of December 21, 2015, the employer did not provide employees working in and around the PSM covered phytosterols process at Arboris, LLC, with flame resistant clothing and require the use of such clothing, thereby exposing the employees to a fire hazard. On or about December 21, 2015, an incident occurred at Arboris, LLC resulting in an overpressurization and flash fire from the release of flammables (Iso-hexane/ethanol). Atlas Industrial Contractors, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.132(a), which was contained in OSHA inspection number 1083583, citation number 1, item number 2a and was affirmed as a final order on October 26, 2015, with respect to a workplace located at 17th street and 2nd Avenue, Huntington, WV 25703.
