29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) On or before March 15, 2016, and at times prior, ASG Energy, LLC., did not ensure that employees were protected against electric shock. Employees of ASG Energy, Inc., were exposed to contact with electrical hazards while installing 120V light fixtures without de-energizing the circuits or effectively guarding the circuits by insulation or other means. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) On or about March 15, 2016, and at times prior, ASG Energy, LLC., did not ensure that portable ladders used to access an upper landing surface had side rails that extended at least 3 feet above the landing surface. Employees of AGS Energy, LLC., were exposed to fall hazards of up to 13 feet while using a portable ladder with side rails that extended approximately 1.5 feet above the landing surface. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
