On 3/14/2016, workers from Puglisi Construction Co., Inc. were working on the exterior elevation near the edge of the roof on a commercial building located at 3889 Mexico Road, St. Peters, MO.   The foreman was observed climbing and standing on the on the guardrails of the basket on a JLG Lift, Model 460SJ, Serial # 10408973, while working on the building.  The foreman was not standing firmly on the floor of the basket while working from the aerial lift.
