(b) Wood Planks: On or about March 15, 2016, and at times prior, employees were exposed to fall hazards from an elevated platform of approximately 24 feet 4 inches to the ground below while constructing a log cabin home.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA: a) Montana Idaho Log and Timber: On or about March 10, 2016, and at times prior, the employer failed to report to OSHA the in-patient hospitalization of one employee within twenty-four (24) hours after a work-related incident.
