(a)  American Civil Constructors, LLC at the intersection of Gold Run Road & Four Mile Canyon Drive, Boulder, CO 80302: On and before March 4, 2016, the employer did not ensure that each employee was instructed in the recognition and avoidance of unsafe conditions associated with the movement of concrete barriers. In the course of attaching a tool (barrier picker) to the bucket of an excavator a concrete barrier was dislodged from its elevated position onto the foot of an employee. This condition resulted in several of the employees toes being crushed.
