29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:    The employer is failing to protect employees from hazards such as but not limited to falls, electrocution, struck by and caught between. This was most recently documented on March 15, 2016 at the jobsite located at 1251 Northwest Briarcliff Parkway in Kansas City, MO.  The employer did not develop or maintain accident prevention programs which allowed an employee to perform work in the area of live electrical equipment.  An employee sustained fatal injuries after contacting the energized electrical component.

This was most recently documented on March 15, 2016 at the jobsite located at 1251 Northwest Briarcliff Parkway in Kansas City, MO.  The employer did not initiate and maintain a program which provided for regular inspections of the job site which allowed an employee to perform work in the area of live electrical equipment.  The employee sustained fatal injuries after contacting the energized electrical component.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    The employer is failing to protect employees from electrocution hazards associated with live parts of equipment or the unexpected energization of electrical equipment.  This was most recently documented on March 15, 2016 at the jobsite located at 1251 Northwest Briarcliff Parkway in Kansas City, MO.   Employees were not instructed in the recognition of the hazards associated with working on or around live electrical circuits or equipment and the methods to mitigate those hazards.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    The employer is failing to protect employees from electrocution hazards associated with live parts of equipment or the unexpected energization of electrical equipment.  This was most recently documented on March 15, 2016 at the jobsite located at 1251 Northwest Briarcliff Parkway in Kansas City, MO.   An employee was electrocuted while working with a 277 volt energized conductor that was not rendered inoperative or guarded.
