Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about March 11, 2016, at the above addressed jobsite, an employee performing carpentry operations approximately forty (40) feet above the ground was not protected from falls, thereby exposing the employee to fall hazards.

29 CFR 1926.701(b): Reinforcing steel. All protruding reinforcing steel, onto and into which employees could fall, shall be guarded to eliminate the hazard of impalement: a. On or about March 11, 2016, at the above addressed jobsite, an employee conducting carpentry operations was exposed to ends of rebar that were not guarded, thereby exposing the employee to the hazards associated with impalement.
