29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels:    Regrind room: On or about April 7, 2016, and times prior thereto, the employer did not perform annual audiograms for employee(s) who were exposed to continuous noise at a dose of 74.2% (8-Hour Time Weighted Average sound level of 85 dBA) during the 429 minute sampling period while performing plastics grinding processes. For a full eight hour work shift this exposure is equivalent to an average sound level of 87.8 dBA. Exposure calculations include a zero increment for the 51 minutes not sampled.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    Assembly, Vacuum Forming: On or about March 15, 2016, and times prior thereto, employee(s) who perform adhesive spray operations were exposed to chemical hazards from the spray application of Penguin Cement 378U (Ethyl Acetate 40-50%, Toluene 10-20%, Cyclohexane 5-15%, Methanol 1-5%, n-hexane 1-10%), a skin irritant and permeator, without the use of personal protective equipment, such as chemical resistant gloves.      Toyoda Gosei Texas, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 315627075, citation number 1 item number 1 and was affirmed as a final order on November 3, 2011, with respect to a workplace located at 1 Lone Star Pass.
