29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  During the roofing job site at 300 Country Trail Dr., Lake St. Louis, on March 16, 2016, employees did not use fall protection harnesses or any other fall protection while installing shingles on a two story house (approximately 19 ft eave to ground and 6 to 12 and 12 to 12 roof pitch).

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:  During the roofing job site at 300 Country Trail Dr., Lake St. Louis, on March 16, 2016, at least one employee was using a harness and lanyard attached to a rope lifeline that was nailed to the roof.  There was no designed anchor plate used.  (Approximately 19 ft eave to ground, and 6 to 12 and 12 to 12 roof pitch).

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  During the roofing job site at 300 Country Trail Dr., Lake St. Louis, on March 16, 2016, where employees  did not use fall protection harnesses or any other fall protection while installing shingles on a two story house (approximately 19 ft eave to ground and 6 to 12 and 12 to 12 roof pitch) and where one employee was tied off to a rope lifeline that was nailed to the roof, training according to the requirements of the standard had not been provided.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  During the roofing job site at 300 Country Trail Dr., Lake St. Louis, on March 16, 2016, where employees  did not use fall protection harnesses or any other fall protection while installing shingles on a two story house (approximately 19 ft eave to ground and 6 to 12 and 12 to 12 roof pitch) and where one employee was tied off to a rope lifeline that was nailed to the roof, training according to the requirements of the standard had not been provided.  There were no certifications of training.
