29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.: On or about 03/12/2016, an employee from Panera, LLC, dba Saint Louis Bread Company was exposed to hazards such as those created by rotating parts of the bagel molder belt system. This caused the employee to be caught-by the system causing an amputation of his right thumb.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. On 03/16/2016, Panera, LLC, dba Saint Louis Bread Company had not ensured cords had proper strain relief on the P.F. Power Factor One forklift battery charger.
