29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:    a.  On or about January 8, 2016, on the southeast side of a 2/12 pitch roof of a new commercial building, two employees installing waterproofing material to the roof were wearing personal fall protection systems incorrectly and were not protected from a 30 feet fall hazard by the use of a personal fall protection system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a.   On or about March 15, 2016 on the northeast side of a new commercial construction building, the ladder used to access the top of the structure did not extend 3 feet above the upper landing surface, exposing employees to a 12 foot fall hazard.
