29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.    At a construction jobsite, employees working beneath overhead truss installation were not wearing head protection exposing them to struck-by hazards by flying or falling objects.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    At a construction jobsite, employees installing roof trusses over 6 feet without fall protection were exposed to unprotected fall hazards up to approximately 12 feet to the ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed.    At a construction jobsite, employees installing roof trusses used two 8 foot Rock River A-Frame step ladders in the closed position to access elevated heights exposing the employees to a fall hazard over 6 feet to the lower deck.
